8 Full-time 1 day ago Full Job Description With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.
This position is located at 2319 St.
Matthews Road Orangeburg SC 29115 Under direct supervision of the Team Leader, coordinate and provide direct nursing interventions, teaching, and community outreach services.
This individual will be a member of a team to identify mentally ill patients in outlying communities who are resistant to receiving mental health services.
The priorities include:
identification, stabilization, and to get the client involved with programs that enhance the quality of life.
Provide medication monitoring and administration service; evaluate compliance and the effectiveness of medication regimes prescribed for mental and physical health; observe, identify and record symptoms or findings; encourages continuation of treatment to help clients maintain community tenure.
Conduct assessments for detection of psychiatric or medical symptoms through observation and record analysis; formulate recommendations, consult and coordinate treatment strategies with staff; assist with devising a plan of care for outpatient TX in crisis situations & assist with implementing alternatives to prevent inpatient state psychiatric admissions.
Coordinate case management activity to link clients to medical or other health care networks in the community for preventive or rehabilitative services based on needs or conditions assessed, in the client's natural environment and meet with family members.
Plan and implement medication education services for clients and their families in their homes.
Evaluate initial medication needs of clients during aftercare follow up appointments upon discharge from hospitals; under supervision and orders per a psychiatrist, review laboratory outcomes for medications that have specific protocols for blood work; dispositions critical findings with the psychiatrist for recommended actions, initiate contact with clients and health care providers as required based on recommendations given per doctor's orders.
Assures that the nursing bags contain medications and are properly stored, dated, secured, and medical paraphernalia is available.
Participates in multi-disciplinary treatment team; assures that nursing care strategies and integrated into TX plans for applicable clients; gives input into clients' status and/or treatment outcomes; functions as a resource to other disciplines regarding medical issues for persons served.
State Agency:
Graduation from an accredited school of nursing and related nursing experience; licensure as a registered nurse by the SC Board of Nursing; certification as a generalist in a related specialty area by the ANA.
DMH Agency:
A bachelor's degree in nursing and one year experience; or a three year diploma and one year experience; or an associate degree in nursing and one year clinical experience in psychiatry, geriatric or medical-surgical nursing, as appropriate to the mission of the employing facility.
Must be licensed as a registered nurse by the SC Board of Nursing.
