Pursuing Excellence, Inspiring Innovation, Promoting Teamwork, Embracing Service and Advancing Equity Through direct community outreach, the Community Health Worker (CHW) will be responsible for helping populations at risk of chronic diseases, navigate access to care and the healthcare system.
CHWs will work closely with community partners to provide culturally appropriate health education and information, give social support and informal counseling, and advocate for individuals and community health needs.
CHWs build trusting relationships with their clients and strive to improve the health of the patients they serve.
CHW certification and other specific program training will be provided after hire.
Assists the population at high risk of chronic diseases and other health issues by reducing barriers to access to care, and other services.
Works within their community and group class settings to help clients manage their own health including promoting health and health equity.
Assists residents in navigating the health care system.
Facilitates assistance or interventions for clients between care visits or programs.
Collaborates with other care team members as needed to address issues outside the scope of practice.
Develops strategic partnerships in the community to support community outreach and referrals.
Establishes peer-to-peer relationships with community partners in order to function as a link to the community.
Acts as an advocate for patients by staying current on community resources.
Provides approved resources to patients to improve their health knowledge and self-sufficiency, help them understand their health condition(s), and develop strategies to improve their health and well-being.
Serves as a liaison between the community and DHEC, as well as patients and the clinical care team.
Reports back to their supervisor on encounters and the results of such encounters.
Appropriately recognizes the need to escalate situations and questions to the supervisor, as needed.
Other duties as assigned State Minimum Requirements:
A high school diploma and relevant program experience.
Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.
Agency Additional Requirements:
A high school diploma or GED and at least one (1) year of experience working in a community-based setting, including paid or volunteer activities with local community-based organizations.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
